Name: What's in a name? Quite a lot, actually. A name for your character is the first place you start, but it can say a lot: what's your character's heritage? What does their name say about them? Do they like their name, or do they dislike it?
Age: Simple enough, but it's here where you decide between student and adult.
Birthdate/place: Remember, the current year is 2038. And don't forget the place!
Species: This can get a bit tricky, beyond witch or wizard. Play around with it, but talk to staff if you're unsure.
Gender: What is your character's gender?
Family: Include blood status and any family members.
Appearance:

Eyes:
Build/height- What does your character look like? How do they carry themselves? Not everyone's tall, dark and handsome: some people are slight and awkward, some are stubby, some have knobby knees. Play around with it!
General Description- This is where you describe skin, face, clothing, and general demeanor. Clothing can tell a lot about a character, so be specific!
Possessions- Anything particular your character owns? List it here! No need to get super-specific, but try to mention any pets, if your character owns a home... you know, major things.
Personality:
Temperment/Mental Attributes-What is your character like? What do they think about? How do they relate to other people? What are their interests, pet peeves? What have past instances done to affect your character mentally? These are all excellent questions to answer here!
Alliance- Are you a 'good guy', a 'bad guy', or somewhere in the grey area!
Alignment- [Lawful Good, Lawful Neutral, Lawful Evil, Neutral Good, Neutral, Neutral Evil, Chaotic Good, Chaotic Neutral, Chaotic Evil] Confused? Read this!
Employment&Education: This section is split into a Student section and an Adult section - feel free to delete the section that doesn't apply to your character.

House- Gryffindor, Slytherin, Hufflepuff, Ravenclaw. Or did you not go to Hogwarts?
Year- 1-7
OWLs/NEWTs- Feel free to generalize.

Job- This isn't only the things in the Positions Roster, this can be anything! Come up with your own job if you don't find the provided ones satisfactory.
Employer- List who's paying you, unless you're self-employed.

Magical Skills- This is where you would list any specific magical talents or abilities, such as Occlumency, Metaphorphagus, Animagus, Werewolf, etc. Don't go crazy here! Stick to no more than 3 or 4, if any. It's fine if your character doesn't have any extraordinary abilities at all!
Patronus- very, very few students can create a full-bodies patronus. Keep that in mind.
Wand- How does your character's wand relate to them? The Wand chooses the Wizard, after all!
Goals/Aspirations- Optional, but very useful in plotting and RP. It's much easier to write a character if you know what they want!
Other Information- Does your character speak any other languages? Do they have a penchant for any muggle talents? Can they play an instrument? List that here!
History-
History varies depending on character, but there should be at least a brief description of childhood, a section about Hogwarts (if you're a graduate. If you're still a student, Hogwarts up until the present), and a section covering from your graduation to the present. Get into detail here! It's a character's history that makes them into who they are, so the more history, the more to your character!
